Qualifications for a medical marijuana card are 

  • Kentucky resident
  • Having a qualifying medical condition
  • Have no disqualifying felony offense convictions


Currently, the Commonwealth of Kentucky recognize the following conditions : 

  • Have one (1) of the qualifying medical conditions, which are:
    • Any type or form of cancer regardless of stage;
    • Chronic, severe, intractable, or debilitating pain;
    • Epilepsy or any other intractable seizure disorder;
    • Multiple sclerosis, muscle spasms, or spasticity;
    • Chronic nausea or cyclical vomiting syndrome that has proven resistant to other conventional medical treatments; and
    • Post-traumatic stress disorder


  •  Visit a medical cannabis practitioner to obtain a written certification.  Your initial visit is required to be in person.  Follow up visits may be telehealth as appropriate.  
  • Submit an Application:   A qualified patient must apply online through the patient and caregiver registry portal or submit a paper application and provide:

                                • Caregiver information, if applicable 

                                • Written certification 

                                • Signed and notarized signature form 

                                • Application fee ($25)

  •  Receive a Registry ID card

                                The Cabinet will approve or deny the application within 30 days of receiving a completed application. If the application is approved, the qualified patient will receive a digital registry ID card, which must be printed or saved on a mobile device, or a printed registry ID card if requested. R 


Currently, no.  Visits are self pay.


Your medical history and a physical exam will be obtained, much like it would  for any other office visit you've had before.  You and your provider will discuss your qualifying condition, medical history, and current medications.  It would be helpful to have medical records, though this is not required.  More importantly, you and your provider will talk about your history and develop a plan for management  Other testing, like a pregnancy test, may be recommended during your visit.


Your initial visit is $175, due at the times of your visit.
